Working Certification Application

Students who are interested in requesting working papers must submit the following required documentation:

1. Form 1 - Working Paper Application (Part I, II and III must be filled out on the form)
2. Form 2 - Doctor Certification of Physical Activity
3. A copy of your Birth Certificate, State ID or School ID (must have your DOB listed)

When this is complete, please send your request through the Working Papers Request Form​​​​​​​.

Please allow 48 hours for a response to your application.

Also, please note that failure to submit the completed documentation will result in a delay in obtaining your working papers.
